Abstract:

Aim — to determine the importance of pregravid preparation (PP) in improving the efficacy of IVF. Material and methods. We examined 122 patients younger than 37 years with a history of two or more unsuccessful IVF attempts: 60 patients who underwent adequate PP (group I) prior to embryo transfer; 62 patients who did not undergo adequate PP (group II). The pregnancy progressions and outcomes in the two groups were evaluated. Results. PP eliminated or reduced the signs of chronic endometritis, and normalized the hormonal profile, blood rheological properties and the condition of the endothelial wall. Patients who underwent adequate PP (Group I) had, on average, an endometrial thickness of 10.66±228 mm, compared to an average endometrial thickness of 8.13±1.42 mm in Group II patients. Group I patients also had better hemodynamics in both the large and small blood vessels. The pregnancy rate for Group I patients was 51.7%, compared to 32.3% for Group II patients, while the ‘take home baby’ index for Group I and Group II patients was 36.7% and 17.7%, respectively. Conclusion. PP can improve the efficacy of IVF in infertile patients by improving the receptivity of the endometrium.
