The paper analyzes the published results of recent researches dealing with the problem of pyelonephritis in pregnant women. Two major factors contributing to the occurrence and development of pyelonephritis are identified in pregnant women; these are a focus of infection and impaired urodynamics of the upper urinary tract. The status of cellular and humoral immunity is considered in detail in patients with pyelonephritis: impaired phagocytosis and scarce quantitative and functional parameters of T-cell mediated immunity, elevated IgA and IgM levels, and changes in the cytokine status. Considerable shifts in the immune status in pyelonephritis serve as the basis for using immunoactive drugs in combination treatment. In pregnant women, the clinical manifestations of pyelonephritis may overlap the symptoms caused by pregnancy complications. Attention is drawn to the use of the algorithm elaborated in our country to examine pregnant women with pyelonephritis in order to clarify the diagnosis. The principles and specific features of treatment in pregnant patients with pyelonephritis are set forth in relation to the duration of pregnancy. The authors declare no conflicts of interest.
