The review presents the tactics of pregnancy management and delivery in the conservative curable stage of chronic kidney failure (CKF). The major factors that determine obstetric outcome and the likelihood of postpartum progressive CKF are emphasized to be baseline renal function and arterial hypertension (AH). The stages of chronic kidney disease (CKD) are determined by the glomerular filtration rate that correlates with serum creatinine levels. In Stages III and IV CKD, CKF is conservative curable. To determine whether pregnancy may be planned and prolonged in patients with CKF is one of the most complicated problems that obstetrics/gynecologists and nephrologists can face. In Russia, the indications for artificial termination of pregnancy in these patients are stipulated by the Ministry of Health and Social Development Order No. 736 dated December 3, 2007, that establishes the maximum allowable serum creatinine level of 200 µmol/l. The paper outlines the principles in the medical treatment of pregnant women with CKF. At the same time, attention is given to the specific features of therapy for pregnant women with CKD added by gestosis, urinary tract infections, or fetoplacental insufficiency. The method of delivery in CKF is stated to depend on the addition of these or those obstetric complications.
