Autoimmune gastritis (AIG) is an unknown disease that results in the destruction of the oxyntic mucosa, mucosal atrophy and metaplasia of the gastric corpus and fundus, leading to impaired production of hydrochloric acid and intrinsic factor, and associated with the increased risk of iron deficiency and B12-deficiency anemia and gastric neoplasia. Early diagnosis of autoimmune gastritis is complicated due to the abcence of specific symptoms and endoscopic changes at the non-atrophic stage of the disease. The earliest and the most common clinical manifestation of AIG is anemia due to iron and vitamin B12 deficiency, but the most danger complication of AIG is the development of gastric neoplasia. This review presents an algorithm for the diagnosis and treatment of AIG based on a current knowledge about of the disease pathogenesis, and the results of clinical trials that are consistent with the principles of evidence-based medicine.
