The development of acute salpingitis and salpingo-oophoritis in the absence of treatment and in patients with an immunodeficiency disorder can lead to the formation of a tubo-ovarian abscess. The peak incidence (4-12%) occurs at the age of 17-28, which is associated with sexual activity and a low-frequency usage of barrier methods of contraception. The article describes a clinical case of a 16-year-old girl with the following diagnosis: exacerbation of chronic salpingitis and oophoritis, tubo-ovarian formation on both sides, adhesions of the pelvic organs and the abdominal cavity. The patient was receiving immunosuppressive therapy for health reasons due to a severe case of juvenile rheumatoid arthritis, which led to an increased risk of infectious complications. During the primary hospitalization in the department anti-inflammatory therapy was carried out, which resulted in a reduced tubo-ovarian formation on the left side and a complete regression of the formation of the right ovary. The girl was again admitted to the department to determine whether surgical treatment was needed. Separation of abdominal adhesions and adhesions of the pelvic organs were carried out, along with evacuation of the purulent contents of the tubo-ovarian formation on the left side, after which sanitation and drainage of the abdominal and pelvic cavities were performed.
