Mural pregnancy is a rarest form of ectopic pregnancy which develops within the myometrium separate from the endometrial cavity and Fallopian tubes. Mural pregnancy may be a cause of uterine rupture, placenta accreta, abundant hemorrhage and hysterectomy in II-III trimester. For these reasons diagnosis should be made early. We present a case of progressing intramural pregnancy in a patient with two previous artificial abortions and hysteroscopic removal of a submucosal fibroid. Her sole symptom was amenorrhea. Diagnostic laparoscopy, performed when β-hCG level exceeded the discriminatory zone, and the gestational sac in the uterine cavity was not detected by transvaginal sonography (TVS), was inconclusive. Five days after, under β-hCG level increase, TVS findings lead to suspicion of interstitial tubal pregnancy. During the second laparoscopy no pathology of uterine tubes was found. Diagnostic hysteroscopy revealed severe intrauterine adhesions. Suggesting uterine pregnancy, suction curettage was performed; no intrauterine pathology was found thereafter. The β-hCG level continued to increase, and finally a pregnancy, located within the myometrium of posterior uterine wall, was detected six days after by TVS. Mural pregnancy was successfully removed laparoscopically with the use of temporary occlusion of ascendant brunches of uterine vessels. The discussion of this case report is supplemented with a review of 71 cases of mural pregnancy published over the past 30 years.
