Objective — to compare the effectiveness and compliance of combined treatment of infertile women with endometriosis with the use of progestogen (dienogest) and gonadotropin releasing hormone agonist (aGnRH). Material and methods. The study included 62 patients. The inclusion criteria were infertility, histologically confirmed endometriosis, the patient’s desire to participate in the study and comply with the treatment protocol. Criteria for exclusion were age less than 18 and more than 45 years, male factor of infertility, anovulation, blocked tubes, severe extragenital diseases. Randomization performed by the «envelopes method. Group I consisted of 42 patients, group II included 20 patients. Group I after surgery has been administered GnRH-abuserelin 3.75 mg or 3.75 mg Diferelinonce in 28 days for 3 cucles, group II received progestogen (dienogest) in a dose of 2 mg per day for 6 months. We used clinical examination, special gynecological examination, ultrasonography (Aloka SSD 3500, Japan), laparoscopy and hysteroscopy («KARL STORZ», Germany). Results. Combined treatment of endometriosis in infertile patients with surgical method (coagulation and excision) and hormonal treatment with aGnRH or dienogest is effective in 1/3 patients and had no differences depending on the variant of hormonal therapy. The frequency of developing pregnancy in Group I was 11.9%, in group II — 5%. The effectiveness of the two approaches in the treatment of endometriosis symptoms was high in both groups, but the incidence and severity of adverse events was more significant in the group receiving GnRHa (90.5%) in comparison group with dienogest (15%). Conclusions. Combined treatment of infertile patients with endometriosis with 2 mg of dienogest is effective and safe.
