A large number of studies are presently being conducted, which demonstrate the high efficiency of checkpoint inhibitors in treating tumors with deficient mismatch repair (dMMR). Microsatellite instability is associated with dysfunction of the MSH2, MLH1, PMS2, and MSH6 genes that normally repair unpaired DNA nucleotides. Deficiency of one or more MMR proteins leads to an inability to control microsatellite lengths during replication. Clones with different lengths of these repeats appear gradually during the process of cell division with dMMR. This phenomenon is called microsatellite instability (MSI). According to various sources, MSI occurs in about 30% of cases of endometrial cancer. The paper describes a case of a complete response to immunotherapy with pembrolizumab in a patient with metastatic endometrial cancer with MSI-high (MSI-H) after progression on two treatment lines. A complete response has been achieved following the fifth pembrolizumab injection; immunotherapy is being continued. No adverse reactions were observed after 11 injections.
