Anaplastic large-cell CD30⁺ skin lymphoma (CD30⁺ ALCL) belongs to the group of primary T-cell skin lymphomas. Clinical presentation includes solitary or clustered, sometimes rapidly growing nodules localized on the skin of the limbs or oral mucosa and prone to necrosis and spontaneous regression in some cases. The unpredictable CD30⁺ ALCL course complicates the diagnosis and provokes debates about the need to treat patients and the choice of therapy. Thus, in single lesions, surgical excision or radiotherapy is performed; in the case of extracutaneous lesions, polychemotherapy is indicated. In multiple massive lesions in the skin only, chemotherapy is a matter of debate. Some authors suggest using low-dose methotrexate in combination with retinoids and interferon-α for treatment. We describe a case of a 56-year-old male successfully treated for CD30⁺ ALCL with systemic interferon-α and an imiquimod-based cream. Six months of therapy resulted in the improvement of the skin lesion, its full epithelialization, and clinical recovery.
