To analyze diagnostic accuracy of instantaneous wave-free ratio with «virtual stenting» for assessing severity of coronary stenoses in patients with diffuse lesions of coronary arteries.
The study included 57 patients with chronic coronary artery disease, diffuse and/or multilevel coronary lesions and iFR ≤0.89. «Virtual stenting» with analysis of apparent iFR before PCI was performed in 59 coronary arteries. Moreover, balloon angioplasty with stenting was followed by assessment of actual iFR after PCI.
We obtained similar apparent iFR before PCI (0.94 [0.92; 0.95]) and actual iFR after PCI (0.91 [0.90; 0.93]). Mean difference between these values in the overall group was 2.3±3.4%, for multilevel lesions — 2.1±3.3%, for diffuse lesions 2.8±3.7%. Absolute percentage error in the overall group was 2.21% (95% CI 1.61—3.28) and did not exceed the 95th percentile (8.11%). This finding indicates adequate accuracy of indicators predicted by iFR angiocoregistration.
Angiocoregistration of iFR with «virtual stenting» can be used for preliminary assessment and modeling of results of elective PCI in patients with multilevel and diffuse coronary lesions.
