Эпилептогенез и функциональная лабильность генома

Резюме

Генные влияния на формирование эпилепсий многообразны. Прежде всего, они направлены на изменение активности ионных каналов и обеспечивающих белков (в том числе синаптических). Эпигенетическое регулирование активности генома также рассматривается как один из важнейших факторов формирования и единичного пароксизма, и эпилепсии как заболевания. На настоящий момент описано огромное количество ассоциаций между генными нарушениями и формированием эпилепсий. При этом они выявляются в генах самой разной детерминантной активности, и одни и те же клинические формы, такие как синдромы Веста, Драве, Отахара, Леннокса—Гасто, могут реализовываться при поражении различных генов. В рамках одной генно-ассоциированной патологии может встречаться большое количество различных типов мутаций, и различным их типам соответствуют различные клинические особенности заболевания. Более того, поражение одного гена может индуцировать несколько механизмов эпилептогенеза (нарушение рецепторной регуляции, проницаемости гематоэнцефалического барьера, нарушения нейрональной дифференцировки). Эпигенетические механизмы обеспечивают функциональную лабильность генома посредством изменения доступности отдельных участков ДНК без внесения изменений в ее структуру. Механизмами влияния на геном в эпилептогенезе являются метилирование генов, кодирующих потенциалзависимые кальциевые и калиевые каналы, генов, оказывающих влияние на нейрональную пластичность, функционирование сигнальных путей, процессы нейровоспаления. Ацетилирование гистонов может рассматриваться в качестве как фактора поддержания и прогрессирования эпилептического процесса, так и «агента быстрой реактивности» при эпилептическом статусе. Некодирующие РНК являются регуляторами таких процессов, как нейровоспаление, апоптоз, синаптогенез, синаптическая пластичность, аномальный спрутинг и ремоделирование нейронных сетей. Круг замыкается патогенными вариантами генов, детерминирующих синтез эпигенетических эффекторов. Расширение объема знаний о механизмах обеспечения лабильности генома является одним из условий успеха в поиске новых путей терапии эпилепсии.
