The aim — is to improve the results of treatment of patients with postpartum endometritis through the use of the molded carbon sorbent VNIITU-1 modified with polyvinyl pyrrolidone (PVP). Material and methods. The study included 109 women with postpartum endometritis who received antibiotic therapy, taking into account the sensitivity of microflora. The main group consisted of 48 patients — 35 women with endometritis after cesarean section, and 13 with endometritis after spontaneous vaginal delivery. The comparison group included 61 patients with postpartum endometritis, including 36 puerperal women after cesarean section and 25 women after spontaneous vaginal delivery. Along with antibacterial therapy, the patients of the main group had molded carbon sorbent VNIITU-1 HDPE injected into the uterine cavity. Standard antibacterial therapy was performed in the comparison group. All patients underwent standard clinical and laboratory examinations, imaging, morphological examination of biopsies and postoperative material, and bacteriological examination of uterine cavity aspirate. Protein fractions in the blood of the puerperal mothers were determined by capillary electrophoresis; the concentration of cytokines in the blood serum and aspirate from the uterine cavity was determined by enzyme-linked immunoassay. Results. Among the microorganisms causing purulent-septic postpartum complications were primarily: Escherichia coli — in 27.5%, Enterococcus faecalis — in 20.2%, Staphylococcus epidermidis — in 16.5% of observations. In the endometrial aspirate of the main group, the concentration of TNF-α decreased by 5.9 times (p<0.05), IL-1β — by 5.6 times (p<0.05); in blood serum, there was a significant decrease in the concentration of IL-1β by 2.5 times, the number of leukocytes by 1.4 times and neutrophils by 3 times; no significant decrease in these parameters was observed in the comparison group. In the main group, a hysterectomy was performed on one patient, and in the comparison group, on 14 patients. The main diagnostic criteria for the transition to surgical treatment were revealed in the main group by ROC-analysis: a reduction of the albumin level of less than 23.1 g/l, an increase of the inerleukin-10 level in serum of more than 47.44 pg/ml, a decrease of the inerleukin-10 level in the exudate from the uterine cavity of less than 28.47 pg/ml, an increase of γ-globulin level of more than 13.4 g/l. Conclusion. The complex approach to treatment of patients with postpartum endometritis with intrauterine use of the molded carbon sorbent VNIITU-1 HDPE allowed for the reduction of the number of hysterectomies by 11.3 times in comparison with traditional antibacterial therapy.
