Abstract:

Objective — to investigate the early neonatal period and to optimize management tactics in the neonates born to mothers with cerebrovascular diseases. Subject and methods. At the Department of Neonatology, the investigators studied the early neonatal period in 104 neonates (2 twins) born to 102 mothers with cerebrovascular diseases at the Moscow Regional Research Institute of Obstetrics and Gynecology in 2005—2016. The analysis of early neonatal abnormalities revealed that the leading pathology was perinatal central nervous system lesion. In this connection, neurosonography and determination of the biochemical markers (S-100 and glial fibrillary acidic protein (GFAP) of brain cell damage were the most informative techniques to clarify the pathogenesis and severity of CNS damage. Results. Thirty-six (34.6%) neonates were born through spontaneous delivery; the remaining 68 (65.4%) babies were born via surgical intervention. Ninety (86.5%) newborns with their mothers were discharged home in satisfactory condition. Fourteen (13.5%) infants were transferred to the second stage of nursing. Conclusion. The investigation has yielded data on the features of the early neonatal period and on the monitoring of diagnostic and therapeutic measures in neonates born to mothers with cerebrovascular diseases. The proposed set of current diagnostic measures and differentiated pathogenetic therapy make it possible to improve perinatal and long-term outcomes in this group of children.
