Abstract:

The modern Russian literature contains no descriptions of the course of pregnancy in rare neurological diseases, whereas the number of foreign publications on this topic has been very insignificant in the past 5 years. The given paper is based on these few publications and descriptions of sporadic clinical cases over longer than 5 years. Its purpose was to most completely familiarize practitioners, first of all obstetricians/gynecologists and anesthesiologists/resuscitation specialists, with the characteristics of management and delivery in pregnant women with rare neurological diseases. The focus is on myopathies and syringomyelia. The most common form of muscular disease in adults is Steinert’s myopathy characterized by diverse clinical severity, involving the cardiovascular system and occurring with respiratory disorders. In Steinert’s myopathy, there is a high incidence of obstetric complications, such as a spontaneous abortion, polyhydramnios, preterm labor, etc. It is shown that the management of pregnant women with orphan neurological disorders in the presence of their low prevalence may present significant challenges, particularly during their delivery, causing anesthesia-related and neurological complications, as well as poor perinatal outcomes. Not only the choice of a delivery mode, but also the nature of anesthesia in view of the possible side effects of the drugs used, is most important for these patients.
