A current approach to the problem of spontaneous preterm birth is based on an incomplete understanding of the mechanisms of its occurrence, as well as on the sequence of and time for preventive measures that should precede the clinical manifestations of preterm labor. Attention is primarily centered on hospitalization of pregnant women with symptoms of preterm labor and their management aims to suppress uterine activity. This approach does not reduce preterm birth rates, but improves conditions for preterm delivery. As of now, there is no test that could accurately predict preterm birth and determine all further actions. Although the currently accepted obstetric guidelines have already resulted in a decrease in perinatal morbidity and mortality rates, an entire scientifically sound strategy must be elaborated to prevent preterm birth on the basis of objective diagnostic markers and effective preventive measures. The aim of this review is to critically analyze current approaches to predicting and preventing preterm birth. The authors declare no conflicts of interest.
