Objective — to evaluate the efficacy of heparin and antiaggregants used to prevent complications and to improve pregnancy outcomes in women with Stages I—III chronic kidney disease (CKD). Subject and methods. The study included 66 patients with CKD (40 patients with Stage I, 8 with Stage II, and 18 with Stage III) who had a history of 117 desired pregnancies during which heparin and/or antiaggregants were not used. During the last pregnancy, all 66 prospectively followed up patients received heparin and dipyridamole to prevent preeclampsia and fetoplacental insufficiency. Moreover, two patients took heparin and antiaggregants during their last pregnancies. A study group consisted of 68 cases of pregnancy in the above mentioned women who used heparin and antiaggregants during pregnancy; a comparison group comprised 117 retrospective cases of pregnancy in the same women, during which they did not receive anticoagulants or antiaggregants. Results. In the study group, the rate of favorable pregnancy outcomes was significantly higher than that in the comparison group [66 (97.1%) of 68 versus 58 (49.6%) of 117] (p<0.001). None patient needed continuous renal replacement therapy within 12 months postpartum. The pregnancy outcomes improved in all stages of CKD. Conclusion. In women with Stages I—III CKD, heparin and dipyridamole caused an increase in the rate of a favorable pregnancy outcome — the birth of alive babies and their postnatal survival, without significantly worsening renal function in their mothers during a year postpartum.
