The given review analyzes the results of the latest researches into reproductive and obstetric problems in epileptic women. It considers the issues of reproductive dysfunction associated not only with the disease, but also with the use of antiepileptic drugs (AEDs). Main emphasis is laid on the analysis of publications on investigations of the course of epilepsy in pregnant women. It has been ascertained that pregnancy does not worsen the course of the disease in the majority of patients, those who have undergone pregravid preparation in particular. At present, there is no convincing evidence for a significant increase in the frequency of pregnancy complications in epileptic patients. However, there may be a direct damaging effect of evolving generalized seizures on the fetus; fetal hypoxia due to the development of maternal lactate acidosis in clonic seizures; and intrauterine fetal death in status epilepticus. The woman with active epilepsy needs effective treatment during pregnancy. At the same time, attention is drawn to the increased risk of fetal malformations when the pregnant woman takes AEDs (valproic acid preparations).
