Objective: to develop methods for the pathogenetically sound prevention of obstetric complications, by studying thrombophilia forms of and hemostatic parameters in pregnant women. Subjects and methods. Ninety-eight pregnant women with obstetric complications during the current pregnancy or in their history who were found to have changes in coagulogram readings were examined. A control group consisted of 45 pregnant women with physiological pregnancy and an uncomplicated obstetric history. Hemostatic parameters, thromboelastographic results, and genetic markers of hereditary thrombophilia were studied. Results. The latter was found in 85% of the patients with obstetric complications and it was concurrent with acquired thrombophilias in 55%. Analyzing the structural properties of a formed clot from thromboelastographic findings leads to the conclusion that it is necessary to correct hemostatic disorders in pregnant women with obstetric complications (80% sensitivity and 82% specificity). Conclusion. Antithrombotic therapy in combination with folic acid, group B vitamins and micronized progesterone is a pathogenically sound method for the prevention of poor pregnancy outcomes in women with hereditary or acquired thrombophilia.
