Seventy-four women with internal genital descent concurrent with other gynecological disease were examined. Group 1 consisted of 38 patients undergoing surgery for internal genital descent and prolapse according to the modified procedure based on uterosacral ligaments fixation with a reinforced aponeurotic flap; Group 2 comprised 36 patients undergoing an original two-stage combined operation to fix uterosacral ligaments with an aponeurotic flap. The studies have shown that the proposed modification is physiologically substantiated and technically easy-to-use, fails to increase the time of surgery (as compared to its original analogue), and makes it possible to maintain reproductive function and to restore the anatomy and function of pelvic organs to a certain extent. Its use is not followed by severe postoperative complications and it permits one to reduce the number of recurrences and to improve quality of life.
