Lung cancer is one of the leading oncological diseases in the modern world, and also ranks 1^st in terms of mortality. According to statistics, the 5-year survival rate for metastatic lung cancer rarely exceeds 5-10%. The most modern methods of treating non-small cell lung cancer, allowing to achieve the best result, are represented by targeted therapy and immunotherapy with inhibitors of checkpoints of the immune response. The article shows the management tactics of a patient with StIV squamous cell lung cancer (brain) and high PD-L1 expression without activating mutations. After progression on the standard first-line polychemotherapy regimen, atezolizumab was started as the second-line drug therapy. 97 injections of the drug were performed from March 2019 to January 2025. The patient tolerated the therapy satisfactorily. According to control examinations (contrast-enhanced brain MRI, whole-body PET/CT with 18-FDG), a decrease in the size of the tumor focus, a decrease in its metabolic activity, and the disappearance of brain metastases were noted, which indicated the high effectiveness of immunotherapy as an independent treatment for metastatic lung cancer.
