Thermochemoradiation therapy was performed in 31 patients with laryngeal cancer (Т3-4N0-3M0). Radiotherapy (RT) was conducted in 2 steps using the regimen 1+1 Gy (at a 4-5-hour interval), 5 times weekly up to SOD 52 or 60 Gy at a 2-week interval after exposure to a total focal dose of 30-32 Gy. Local hyperthermia (LHT) was carried out as 3-6 sessions twice weekly before the second fraction of RT. On the days of LHT, the second fraction of RT was increased up to 3 Gy. The courses of polychemotherapy were combined with RT and LHT at the beginning of each step of treatment. In the entire group, a complete response of primary tumor was diagnosed in 25 (80.6%) patients, a partial one was in 6 (19.3%). Out of 12 patients with N1-3, complete and partial responses of metastases were noted in 5 (41.7%) and 5 (41.7%) cases, respectively. Five-year overall survival was 88.2 and 62.1% for Т3N0 and Т4N1-3, respectively. In this period, local control of primary tumor in T3 and T4 was found in 88.2 and 77.8%, respectively; regional control of metastases in N1-3 was in 33.3%. Late radiation laryngeal damages as mucosal edema and perichondritis were diagnosed in 4 (12.9%) patients. Thus, conservative thermochemoradiation therapy for locally advanced laryngeal cancer can yield rather high results comparable with the data of combined treatment.
