The article deals with the issues of treatment and complications resulting from the immunosuppressive therapy for pemphigus vulgaris. The authors emphasize that the use of the aggressive techniques of treatment of patients with pemphigus vulgaris using the systemic corticosteroids, methotrexate, azathioprine, chlorambucil, cyclophosphamide, dapsone, and sodium thiomalate does not improve the overall prognosis of the disease or the quality of life of patients. For this reason, the authors suggest the method of combination therapy of patients with pemphigus vulgaris with concomitant administration of the medium therapeutic doses of systemic glucocorticosteroids and methotrexate in the form of Metodzhekt injected subcutaneously once per week at a dose of 7.5—15.0 mg. The duration of this regimen of combination therapy was 4—5 weeks at a hospital followed by outpatient therapy under the supervision of dermatologist until the complete clinical remission. The article presents out own clinical observations. There were no side effects of the drugs or hematological complications of therapy. The duration of clinical remission in patients with pemphigus vulgaris was 11.7±1.4 months during the outpatient follow-up period after the course of therapy.
