Abstract:

The authors present the novel information about pathogenesis of atopic dermatitis including the role of genetically determined defects of epidermis, immunopathological mechanisms underlying the formation of acute and chronic manifestations of the disease, and the data on the presence of "subclinical" skin inflammation outside the affected areas. The results of evidence-based investigations are reported to substantiate the expediency of the new strategy of "proactive" external therapy for the patients with atopic dermatitis using anti-inflammatory agents (e.g. topical glucocorticosteroids or tacrolimus ointment) in combination with emmolients. This treatment has been shown to enhance the overall efficiency of therapy and prolong the duration of clinical remission of atopic dermatitis.
