OBJECTIVE

To evaluate predictors of acute kidney disease (AKD) in patients with stable angina after coronary artery bypass grafting (CABG).

MATERIAL AND METHODS

The study enrolled 59 patients with stable angina and no underlying kidney disease who underwent CABG. Age of patients was 64 (59—68) years (men — 83%). Carbohydrate metabolism disorders were detected in 44% of patients, arterial hypertension — in 97%, previous myocardial infarction — in 54% of patients. Off-pump CABG was performed in 29% of patients, bilateral IMA grafting — in 49% of patients. Cardiopulmonary bypass (CPB) time was 64 (55—84) min, the number of grafts — 2.7±0.7. Glomerular filtration rate (GFR) was calculated using the CKD-EPI formula. AKI was diagnosed taking into account the KDIGO requirements 2012, AKI in 90 days after CABG — KDIGO requirements 2020.

RESULTS

Acute kidney injury associated with CABG was registered in 10 (17%) patients, AKD in 90 days after CABG — in 8 (14%) patients. In 25% of cases, AKD was associated with postoperative acute kidney injury. In 75% of cases, AKD developed without previous injury.

CONCLUSION

Older age (>70.5 (66—74) years), lower baseline GFR (73 (67—77) ml/min/1.73m²) and longer CPB time (77 (70—103) min) were associated with AKD after CABG. Age increase by 1 year significantly increased the probability of AKD by 45.7%.
