Abstract:

OBJECTIVE

To analyze risk factors of stroke in patients with internal carotid artery stenosis.

MATERIAL AND METHODS

The influence of such factors as unstable atherosclerotic plaque, smoking, alcohol abuse, high cholesterol, diabetes mellitus, arterial hypertension, obesity and optimal drug therapy was studied. Patients were divided into 2 groups: group 1 — 109 patients who underwent ipsilateral carotid endarterectomy within 2—14 days after acute cerebrovascular accident; group 2 (n=622) — elective surgery in 2006—2013.

RESULTS

Unstable atherosclerotic plaques were more common in the 1st group. There were significantly more smokers in group 1; 31 (28.4%) patients in the main group and 41 (6.6%) patients in the control group abused alcohol. Elevated cholesterol levels (>4.5 mmol/l) were observed in 94 (86.2%) patients of the main group and 437 (70.3%) patients in the control group. The number of patients with and without diabetes was similar. The incidence of arterial hypertension was also similar. Obesity was more common in the 2nd group. Optimal drug therapy was significantly rarer in the main group.

CONCLUSION

Significant risk factors of stroke were unstable atherosclerotic plaque, smoking, alcohol abuse, hypercholesterolemia and no optimal drug therapy. Obesity, diabetes mellitus and arterial hypertension did not affect the risk of acute cerebrovascular accident.
