Abstract:

OBJECTIVE

To analyze thromboembolic complications after cardiac surgery associated with resection of the left atrial appendage.

MATERIAL AND METHODS

The study included 114 patients. Patients with permanent atrial fibrillation underwent isolated thoracoscopic resection of the left atrial appendage (n=7, 6%). In patients with persistent forms of atrial fibrillation, resection of the left atrial appendage was performed within the protocol of thoracoscopic radiofrequency fragmentation of the left atrium (n=107, 94%). Oral anticoagulants were prescribed after the second postoperative day for a 3-month period.

RESULTS

Eleven patients (10%) stopped intake of anticoagulants for various non-medical reasons within 90 days after surgery. Four of them developed thromboembolic complications. According to neural network analysis (multilayer perceptron), the most important predictor of postoperative thromboembolic complications was discontinuation of anticoagulation within 90 days after thoracoscopic radiofrequency fragmentation of the left atrium (normalized importance 100%).

CONCLUSION

Resection of the left atrial appendage is an effective self-sufficient method for prevention of thromboembolic complications in patients with atrial fibrillation. However, anticoagulation should be prescribed for at least 90 postoperative days despite resection of the left atrial appendage due to the risk of thrombosis within the area of radiofrequency ablation of the left atrium.
