Severe heart failure caused by thrombosis of mitral valve prosthesis occurred in 56-year-old male in 3 years after surgery. Patient discontinued warfarin intake one month before admission to our hospital due to idiopathic coagulopathy and recurrent hemorrhagic stroke. It was followed by valve prosthesis thrombosis and symptoms of critical mitral stenosis. Dysfunction of mitral prosthesis (stenosis with pressure gradient up to 26 mm Hg) and severe aortic insufficiency were diagnosed. Redo mitral valve replacement with Carpentier—Edwards №25 aortic valve prosthesis in inverted position was performed due to unavailable mitral valve prostheses. Simultaneous aortic valve replacement with Carpentier—Edwards №21 bioprosthesis was also carried out. There are no any symptoms within 2 years after discharge.
