Abstract:

Aim. To analyze early postoperative complications depending on type of aortic arch repair. Material and methods. For a 7-year period 126 patients underwent aortic arch repair that was followed by thoracic aortic reconstruction if it was indicated («Hemiarch procedure», total arch replacement, «frozen elephant trunk») under deep/moderate hypothermia and antegrade cerebral perfusion via innominate artery. Results. Re-sternotomy for bleeding was made in 8 (6.3%) cases in early postoperative period. 25 (19.8%) patients had respiratory failure followed by tracheostomy. Acute myocardial infarction in postoperative period developed in 4 (3.2%) patients. Hemodialysis for acute renal failure was performed in 14 (11.1%) cases. Transient ischemic attack was diagnosed in 2 (1.6%) patients. Somatogenic psychosis was noted in 9 (7.1%) cases. 30-day mortality was 7.1% (9 cases) and in-hospital mortality was 9.5% (12). Conclusion. The quality and quantity of early postoperative complications depend on type of aortic arch repair. Reduced time of cardiopulmonary bypass and circulatory arrest is followed by decrease of postoperative complications incidence and mortality rate. Hybrid treatment of thoracic aortic pathology is characterized by significantly lower early mortality compared with conventional open aortic arch replacement.
