Colonic obstruction is frequently caused by colon cancer. Surgical treatment of acute intestinal obstruction is associated with frequent postoperative complications and high postoperative mortality, reaching 25%. Endoscopic stenting of tumor strictures is an upcoming trend in this field.

AIM

Compare the effectiveness of the malignant colonic obstruction (MCO) treatment by stenting and defunctioning colostomy with interval resection.

MATERIAL AND METHODS

356 patients were included in the retrospective analysis between July 2015 to November 2019. All patients were admitted urgently to the S.P. Botkin City Clinical Hospital with MCO. Patients were divided into the main group (I, n=210), in which the first stage of treatment was stenting, and the control group (II, n=146), in which resection with D2 lymph node dissection was performed as the first stage and followed by formation either a primary anastomosis or a colostomy.

RESULTS

Postoperative mortality in the control group was 11.0% (n=16) at the first stage of the treatment and 2.05% (n=3) at the second. After stenting in the main group, postoperative mortality was 1.67% (n=2), after the second stage (surgical treatment) — 0%. In the main group, the tumor recurrence during the first year developed in 4.9% in the control group in 12.4%; however, the differences were not statistically significant. One-year mortality in the control group was 20% and 13.6% in the main group (p<0.05).

CONCLUSION

Stenting in MOC showed a lower postoperative and one-year mortality rate compared to urgent surgical interventions.
