Neuroprotective mechanisms of delay of neurodegeneration and recovery of function in multiple sclerosis are reviewed. Different adaptive reactions (neuroprotective autoimmunity, neuroplasticity, sodium channels expression and function changes, remyelination) are described. The possibility of neurogenesis, axonal regeneration and synaptic changes is discussed. Understanding the compensatory mechanisms in multiple sclerosis is a fundamental basis for the development of new therapeutic approaches.
