The paper analyzes the data available in modern literature on natural fertility in women. Fertility is a complex physiological and behavior-determined phenomenon. Natural conception and pregnancy are due to age. The female reproductive system differs from other body systems in that it has a higher aging rate. Studies over the last few decades have greatly expanded knowledge about the reproductive system of a woman, which could formulate ideas about her biological age from the ovarian status (ovarian reserve). The ovarian reserve forms the basis of a reproductive potential and defines the functional reserve of the ovaries, their ability to produce a healthy follicle with a mature ovocyte, the possibility of an adequate response to hormonal stimulation in the assisted reproductive technology programs. There are a large number of markers for assessing the ovarian reserve and more than 20 models for predicting the outcomes of assisted reproductive technologies. There is a need to comprehensively evaluate the reproductive system to decide what measures should be taken for the further management of diminished ovarian function. The authors declare no conflicts of interest.
