Arterial hypertension (AH) is a leading modifiable risk factor for the development of diseases of the circulatory system. In Kuzbass, according to a study by ESSE RF (2012—2013), the prevalence of hypertension exceeds the average for Russia. One of the main tasks of the outpatient-polyclinic service of the Kemerovo region is maximum coverage of the population with AH by dispensary observation.
OBJECTIVE
To create and implement organizational methods of dispensary observation of patients with arterial hypertension (AH) using on-line monitoring technology.
MATERIAL AND METHODS
The study included 53 patients with previously installed AH, and the preventive surveillance method was developed in accordance with national recommendations of diagnosis and treatment of patients with AH. In addition to achieving the target blood pressure (BP) level, the study criteria were a decrease in the frequency or cessation of hypertensive crises (patient stay in the “red zone”), a decrease in maximum systolic blood pressure, no development of hypertension complications, and the level of compliance with the ongoing preventive program. The feature of the methodology was the main load fell on the shoulders of nurses for controlling of on-line monitoring.
RESULTS AND CONCLUSIONS
As a result of the study, the effectiveness of on-line BP monitoring in reducing the average and maximum BP levels, up to the target values for patients who initially had no target BP levels, and the effectiveness of the method in increasing patients’ adherence to regular BP monitoring, as well as to the drug treatment of BP were proved. The possibility of effective participation in the remote dispensary monitoring of outpatient clinic nurses has been proven.
