Hypertension is one of the most common chronic noncommunicable diseases and the most important risk factor for these diseases and their complications. Objective — to study the time course of changes in the mean systolic and diastolic blood pressures (SBP and DBP) in different age and sex groups in Russia and the United States of America (USA) in 1975 to 2014. Material and methods. Secondary analysis of data from cross-sectional population-based studies in Russia and the USA in 1975—1982 and 2007—2014 was carried out to study blood pressures (BP) in the men and women from different age groups. A total of 48,974 observations were made. Results. According to the update, the mean BPs in the Russian population are considerably higher than in the USA in men and women from all age groups (p<0.0001); the differences in their mean values are 6—17 mm Hg for SBP and 6—14 mm Hg for DBP; these differences increase with age. According to the data obtained 35 years ago, the BP differences were less pronounced with a similar trend. Blood pressure changes over a 30-year period in the USA population are characterized by a significant decrease in SBP and DBP in all the groups; in Russia there was a significant reduction in DBP in men and women and SBP in women. Conclusion. Natural differences were revealed at the population level: ВР in the population of Russia was on average higher than that in the population in the United States. Over 30 years, BP levels declined in both populations; this reduction was more pronounced in the USA.
