OBJECTIVE
To describe a clinical case of endometriosis of two incompetent scars on the uterus after two cesarean sections and our operational experience with laparoscopic correction of this condition.
Patient. A 27-year-old female patient with two previous cesarean sections in history. In March 2019, there was an early termination of pregnancy for medical reasons due to an inconsistent scar on the uterus after a cesarean section. According to the data of transvaginal ultrasound and MRI, two scar defects on the uterus after cesarean section were revealed at different levels — in the isthmus and 15 mm above the level of the internal pharynx measuring 17.0±13.6 mm with a residual thickness of the myometrium above the defect 2.6 and 2.9 mm.
RESULTS
The postoperative course was uneventful, and the patient was discharged on the 4th day after surgery. After 30 and 60 days of postoperative follow-up, transvaginal and transabdominal ultrasound showed complete anatomical restoration of the uterine defect. According to the results of histological examination of the material of thinned scars on the uterus, signs of endometriosis were revealed in the area of both scars on the uterus.
CONCLUSION
Positive reproductive results were noted after surgical treatment of thinned scars on the uterus after two cesarean sections. Laparoscopy allows you to completely restore the defect in the uterus, thereby restoring the normal thickness of the myometrium. Therefore, as demonstrated in this case, the laparoscopic approach can be considered the procedure of choice for repairing a large myometrial defect with thinning of the residual myometrium in the presence of more than one inconsistent uterine scar.
