Abstract:

Polycystic ovary syndrome (PCOS) is one of the common endocrinopathies. According to modern literature, PCOS is detected in every tenth woman of reproductive age, accounting for up to 90% of all forms of hyperandrogenism. Sclerocystic ovaries were described by Stein and Leventhal in 1935 as a syndrome of amenorrhea and enlarged ovaries, combined in 2/3 of cases with hirsutism and in every second case with obesity. Since the early 2000s, the existence of a wide variety of phenotypes of the syndrome has been noted, which manifests itself as a significant variation in the clinical picture of the disease, endocrine profile and morphological signs. A surgical method for the treatment of PCOS was proposed by Stein and Leventhal (and, according to some data, by domestic authors Ya.K. Khachkuruzov and S.K. Lesnoy) in the form of a laparotomy and bilateral wedge-shaped resection of 1/2 or 3/4 ovarian tissue, after which they were observed normalization of menstrual function and pregnancy in patients with this syndrome. The evolution in the treatment of PCOS also affected the surgical approach: instead of laparotomy, they switched to laparoscopic, and the wedge-shaped resection (due to the negative effect on the ovarian reserve) was replaced by ovarian drilling. According to the 2018 international clinical guidelines for PCOS, the surgical treatment is a second-line therapy for women with PCOS with clomiphene resistance, anovulatory infertility and the absence of other infertility factors in the couple.
