Abstract:

This article presents a description of a rare clinical case of a malignant variant of solitary fibrous tumor of the pleura with paraneoplastic Doege-Potter syndrome in a 74-year-old male. The disease was detected due to acute episodes of hypoglycemia. Evaluation revealed a massive intrapleural non-epithelial tumor in the right hemithorax, measuring up to 26 cm, accompanied by hypoglycemic episodes as low as 2.2 mmol/L during the night. Diagnosis was confirmed morphologically through core biopsy. The patient underwent radical surgery at the Thoracic Department of the Moscow Multidisciplinary Clinical Center «Communarka,» which included right thoracotomy, extrapleural tumor removal with atypical resection of the upper and lower lobes of the right lung. After surgical treatment, the hypoglycemic syndrome was resolved within a day. Histological and immunohistochemical analysis of the surgical specimen confirmed the malignant variant of solitary fibrous tumor of the pleura with invasion into the lung parenchyma.
