Abstract:

OBJECTIVE

To study nutritional status indicators in palliative care patients with malignant neoplasms (MNP) in relation to the nature of therapeutic comorbidity.

SUBJECTS AND METHODS

A one-stage study was conducted in 74 palliative care patients with cancer. All the examinees underwent an integral assessment of their nutritional status and clinical and laboratory parameters according to the presence of chronic non-communicable diseases (CNCDs).

RESULTS

High comorbidity rates were demonstrated in 52 (70.3%) examinees. At the same time, 18 (24.3%) patients were found to have three CNCDs; 14 (19%) patients had two CNCDs, and 20 (27%) patients had one CNCD. In a subgroup of patients with a burdened therapeutic comorbidity profile, anemia was significantly more frequently observed; there were substantially low serum albumin levels and glomerular filtration rate, and, conversely, considerably high urea and creatinine levels. The interpretation of changes in the body component composition showed a tendency to increase the creatinine-growth index in palliative care patients with MNP and CNCD. The tendency to reduce lean body mass in this cohort suggests that the hypermetabolic syndrome may develop. A multivariate regression analysis revealed a relationship between the presence of CNCD in palliative care patients with MNP and the nutritional status indicators.

CONCLUSION

The integral analysis of trophic state indices revealed the reliable signs of nutritional deficiency in cancer patients in need of palliative care. The concomitance of MNP and a burdened therapeutic comorbidity exacerbates malnutrition.
