Due to the high rates of morbidity and mortality in males, the treatment of patients with prostate cancer (PCa) is an extremely urgent matter. The proportion of patients with localized PCa has significantly increased in Russia over the past 10 years. This fact is explained by that the measurement of prostate specific antigen (PSA) levels in the male population has been widely introduced and that the efficiency of radiation diagnostic methods has been substantially enhanced. The paper presents the results of surgical treatment in patients with high-risk localized PCa.

OBJECTIVE

To assess the long-term oncological results of surgical treatment in patients with high-risk localized PCa.

PATIENTS AND METHODS

The database comprising 1507 patients with localized PCa was retrospectively analyzed. According to the European Association of Urology guidelines, all the patients met the high risk criteria for the disease. All the patients underwent radical prostatectomy with pelvic lymphadenectomy.

RESULTS

The median follow-up period was 59 months. Biochemical recurrence was diagnosed in 228 (35.5%) patients. The 5-year PSA relapse-free survival was 65.7±2.1 months; the 5-year overall and tumor-specific survivals were 94.9±1 and 98.7±0.5 months, respectively.

CONCLUSION

Radical surgical treatment demonstrates satisfactory long-term oncological results. Histological examination of the removed prostate gland and lymph nodes makes it possible to obtain reliable information about the prevalence of the tumor process and to develop the most optimal treatment policy for each patient.
