Complications after thoracoscopic lobectomy in patients with malignant lung tumors

Objective — to improve the immediate results of thoracoscopic anatomical lung resections as lobectomy in patients with malignant primary and metastatic lung tumors, by reducing the incidence of complications, through a systems analysis of the data of a specialized facility. Material and methods. The investigators used the 6-year (2010—2015 inclusive) data of the Thoracic Surgery Department, P.A. Herzen Moscow Oncology Research Institute, which performed 219 thoracoscopic anatomic lung resections (lobectomies), including 47 operations completed via thoracotomic access as a result of conversion. Complications following thoracoscopic surgery and that through conversion to thoracotomy were comparatively analyzed. Results. The overall incidence of complications after completed thoracoscopic operations was similar to that after conversion and amounted to 42 (24.4%) of 172 versus 11 (23.4%) of 47 (p>0.05). The incidence of prolonged postoperative lung parenchymal air leak was lower after thoracotomy, which was 4 (8.5%) of 47 than that after thoracoscopic surgery 25 (14.5%) of 172 (p>0.05. Cardiac arrhythmia was more common following conversion 3 (6.3%) of 47 versus 3 (1.7%) of 172 (p>0.05). Incompetent bronchial stump was not encountered in the conversion group and it was noted in one patient after thoracoscopic surgery. Conclusion. Thus, thoracoscopic lobectomy is a relatively safe surgical intervention. Intraoperative conversion does not lead to postoperative deterioration. The developed technique can achieve effective aerostasis, including that in patients with poorly pronounced interlobular fissures. Age over 70 years is an unfavorable risk factor for postoperative complications.
