Objective — to increase survival in patients with secondary brain lesion and to identify positive and negative prognostic factors. Subjects and methods. Seventy-two patients aged 33 to 72 years with intracerebral metastases from breast cancer were treated in 2008—2013. The patients were divided according to recursive partitioning analysis (RPA) class, number of metastases, and the presence of an extracranial abnormality. Results. Survival rates were estimated in relation to different factors. The median overall survival was 12 months and, according to RPA class, was 12, 11, and 7 months, respectively (p=0.01637). If metastases were in the supra-, infratentorial space or at concurrent sites, the median survival was 10.5, 12, and 7 months, respectively (p=0.27647). According to the number of brain metastases, the median survival was 12, 9, and 4 months in solitary metastases, oligometastases, and multiple brain involvement, respectively (p=0.13939). Conclusion. Overall survival is influenced by factors, such as multiplicity of metastatic involvement, RPA class. The median overall survival of patients who did not receive another treatment option after surgical treatment was as long as 4 months. When combined treatment was used, the survival was 12 months.
