Objective: to analyze breast cancer (BC) recurrences after radical subcutaneous mastectomy. Following the latter, the breast skin, nipple-areola unit, and submammary fold are preserved and the whole gland tissue was removed together with axillary, subclavian, and subscapular lymph nodes. Subjects and methods. Recurrent BC was identified in 32 (4.6%) of the 703 patients over a 15-year follow-up. The time to BC recurrences was 8 months to 10 years. The investigation analyzed local recurrences that included the emergence of a nodule that was structurally similar to the primary tumor and developed within the breast. The diagnosis of recurrent BC encompassed ultrasonography, mammography, and morphological verification. Results. A local recurrence was diagnosed simultaneously with distant metastases in 9.4%. The histological characteristics of the nodule and recurrent BC were identical in 32 cases. The basic recurrence treatment was surgery, when needed, complemented by radiation and drug therapy. Conclusion. Based on new evidence for the subtypes of BC, the data of our investigation may be recommended not to perform radiotherapy after radical subcutaneous mastectomy for nodules 1 cm or less, in the presence of favorable morphological criteria (neither multicentricity, a cancer embolus around the periphery of the nodule and in breast tissue, third-grade invasive ductal cancer, nor triple negative or HER2 BC).
