Objective: to study the capability and efficiency of preoperative thermochemoradiation therapy (TCRT) in patients with locally advanced laryngeal cancer. Subjects and methods. Preoperative TCRT was used in 28 patients with locally advanced laryngeal cancer (Т3—4N0—3M0). Radiotherapy with SOD 32 Gy was performed five times weekly, by dividing the daily dose of radiation into 2 fractional doses (at a 4-hour interval) according to the 1 Gy+1 Gy regimen, on the days of local hyperthermia according to the 1 Gy+3 Gy regimen. Local hyperthermia was carried out as 3—4 sessions twice weekly before the second fractional dose of RT. Polychemotherapy was used concurrently with radiotherapy and local hyperthermia. Organ-sparing surgery was performed in 12 (42.9%) patients. Results. Primary intention healing in postoperative wounds occurred in 21 (75%) patients. The cumulative 5-year overall survival was 89% in the T3—4N0—3 group and 100% in the patients without regional metastases (T3—4N0). In these periods, relapse-free survival was 94 and 90% for patients with T3 and T4, respectively. Recurrent metastases developed in 20% of the patients. Conclusion. Preoperative TCRT is a highly effective treatment for locally advanced laryngeal cancer and fails to lead to severe postoperative complications.
