Abstract:

Peritoneal dissemination is one of the main reasons for radical treatment failure in patients with gastric cancer. Up to 20% of patients have peritoneal metastases at surgery for primary resectable gastric cancer. The median survival is 3.5 months. Cytoreductive surgery in combination with hyperthermic intraoperative intraperitoneal chemotherapy is the only effective treatment for peritoneal dissemination now. The paper describes the first experience of the P.A. Herzen Moscow Oncology Research Institute with hyperthermic intraoperative intraperitoneal chemotherapy in patients with locally advanced and disseminated gastric cancer. It presents the results of international and Russian investigations dealing with this problem. Indications and contraindications are determined; the procedure for surgery is described. The immediate results of treatment are given; complications are characterized; possible ways of improving this procedure used in patients with locally advanced gastric cancer have been elaborated.
