Abstract:

The results of surgical treatment performed in the past 10 years are analyzed in 220 patients with gastric cancer. Gastrectomy with D2 lymph node dissection is now a major surgical volume for gastric cancer. This type of surgery in specialized centers is characterized by low postoperative complication and death rates. D2 lymph node dissection shows a high rate of lymphogenic metastasis, including that to the second lymph outflow collector. The adequate volume of surgical intervention into the stomach and lymph collectors results in low isolated local and regional recurrence rates. In the pattern of progressive gastric cancer, there is a preponderance of distant metastases, among which peritoneal carcinomatosis prevails. An optimal combination of adjuvant systemic (chemotherapy, targeted therapy) and local (radiotherapy) exposures is required to increase the rates of resectability and locomotor control and to prevent distant metastases.
