About 40,000 patients with gastric cancer are detected and 75% are diagnosed with in Stages III-IV disease in Russia every year, which predetermines the poor results of treatment. The diagnosis rate of early gastric cancer (EGC) is not greater than 10%. Most cases of EGC are diagnosed in persons older than 60 years who had a comorbid condition and cannot undergo radical surgery. According to the data of different authors, complete regression of EGC after photodynamic therapy (PDT) is noted in 67-100% of cases and associated with the level of primary tumor invasion. The paper describes a case of successful EGC treatment in very old patients with manifest comorbidity. According to the records of Moscow City Clinical Cancer Hospital Sixty-Two, complete primary tumor regression after PDT is observed in 89% in a group of functionally inoperable patients with early (Tis-1N0M0) cancer of the esophagus, stomach, and large bowel. The duration of a follow-up is 5 months to 5 years.
