Psoriasis is a chronic, immune-associated, multifactorial disease affecting the skin with the risk of complications in other organs. Treatment of psoriasis requires an individual approach in choosing specific local or systemic therapy. Currently, topical therapy is used to treat mild forms, while severe forms require the administration of systemic agents such as methotrexate or biological drugs.

OBJECTIVE

To compare the efficacy of treatment for moderate to severe psoriasis in three patient groups depending on the used type of treatment.

MATERIAL AND METHODS

The study included 155 patients with moderate to severe psoriasis. Patients were aged from 18 to 78 years. Patients in the 1^st group used daily applications of mometasone furoate 0.1% ointment. Patients in the 2^nd group received methotrexate at a dose of 15 mg intramuscularly once a week. Patients in the 3^rd group received the IL-17 inhibitor (netakimab, 120 mg) as two subcutaneous injections at a dose of 1 ml (60 mg) once a week at weeks 0, 1, and 2, then maintenance therapy monthly starting from week 4.

RESULTS

In the 1^st group of patients, the median PASI decreased by 2.78%, and the median BSA index declined by 3.13%. In the 2^nd group of patients, the median PASI decreased by 21.05%, the median BSA index by 33.33%, and the frequency of clinical manifestations of psoriatic arthritis also declined significantly. In the 3^rd group of patients, the median PASI decreased by 73.68%, the median BSA index — by 78.95%; the frequency of active complaints regarding psoriatic arthritis declined significantly from 57.69 to 11.53%.
