The article addresses issues of dermato-oncological care for the Moscow population. We propose an organizational model of this type of care and the scheme for routing patients with skin neoplasms, including the skin malignancy (SM) prevention office (the first stage), the Center of noninvasive skin diagnostics (the second stage), and if malignant nature of the tumor cannot be excluded — a specialized oncological institution (the third stage) of the Moscow Health Department (MHD). As part of this model implementation, a training course for general practitioners was organized at the Moscow Research Center for Dermatovenerology and Cosmetology (MRCDC). The course included skin examination to detect skin cancer, including dermatoscopy, as part of medical examinations and health check-ups for the adult population. In addition, dermato-oncology educational webinars and in-person practical seminars, «Fundamentals of Dermatoscopy,» for dermato-oncologists of SM prevention offices are regularly held. Three years of experience with this model have been analyzed, demonstrating its high efficiency: SN was excluded in 314.600 people or 85% of all those who applied to SN prevention offices. The high performance of dermatologists made it possible to avoid overloading specialized oncological institutions with non-priority patients, increasing their work efficiency. The positive experience gained, and the developed model of dermato-oncological care can be scaled up to other regions of Russia.
