Psoriatic arthritis (PA) is one of the most severe complications of psoriasis. The prevalence of psoriatic arthritis among psoriasis patients ranges from 13.5% to 45%. In recent years, an increase in the incidence of PA and a relative increase in the severe forms in which standard of care proves ineffective were observed. Despite current anti-inflammatory therapy options, PA significantly affects many aspects of patients' lives. Patients with PA have a higher rate of early disability and mortality compared to psoriasis patients without PA. The article describes the experience of a new therapeutic approach to the treatment of PA with a monoclonal antibody drug product. Interleukin-17A (IL-17A) is one of the major proinflammatory cytokines involved in PA pathogenesis. IL-17A is produced by activated Th17-lymphocytes as well as macrophages and fibroblasts. In this regard, the use of IL-17A inhibitors in PA is justified; and netakimab has a high IL-17A binding ability among them. Clinical cases of effective treatment with the drug in patients with severe forms of psoriasis and PA refractory to standard therapies were reviewed.
