Psoriasis is known to have a genetic component with estimated heritability of 66%. Large groups of interacting genes with altered expression, aberration of certain chromosomal areas (CNVs), and loci associated with psoriasis are involved in the pathology manifestation. However, the pathological process is triggered not only by the cumulative effect of these genetic factors but also by external triggers and epigenetic factors. DNA methylation is an important element of the epigenetic regulation. For example, the experimentally established decisive role of gene methylation in the development of psoriasis broadens the range of pharmacological agents, which contribute to the suppression of psoriatic manifestations, due to incorporating drugs involved in the transmethylation reaction. One of these drugs is ademetionine (Heptral) that is able to compensate for deficiency of S-adenosyl-L-methionine (ademetionine) and stimulate its production in the body. Material and methods. The study included 253 patients suffering from different clinical forms of psoriasis. An examination of the patients included general work-up, a dermatological examination and PASI evaluation, an analysis of biochemical blood parameters and clinical laboratory blood parameters, general clinical urinalysis, and X-ray inspection. Patients with signs of the liver pathology were divided into two groups: patients of the 1st group received Heptral as a hepatoprotective drug, patients of the 2nd group received essentiale forte H. Results. All biochemical blood parameters in the group receiving Heptral were reduced to almost normal values, with PASI being decreased in 4.2 times. The aminotransferase (AST and ALT) levels in the group receiving essentiale forte H still remained significantly increased, with PASI being decreased in only 3 times. Evaluation of the dermatology life quality index (DLQI) after the conducted therapy demonstrated an increase in the mean score from 15.6 to 37.2 in patients of the 1st group and from 16.1 to 29.8 in patients of the 2nd group. Conclusions. The study not only demonstrated a high efficacy of Heptral impact on the biochemical parameters of blood (ALT, AST, total bilirubin, etc) but also promoted the positive dynamics of the clinical presentation.
