Psoriasis is currently regarded as chronic erythematous-squamous dermatosis of multifactorial nature developing largely as a result of immunogenetic and lipid disorders. Many authors consider psoriasis to be a result of disturbances in lipid metabolism. However, most of them evaluate these disturbances based solely on the measurement of blood lipid levels regardless of changes in the lipid spectrum at the surface of epidermis of psoriatic patients. Meanwhile, the normal functioning of the skin barrier strongly depends on the structure and composition of intercellular lipids. Lipids of the horny layer of the skin are synthesized in keratinocytes as these cells are becoming mature. The lipids (or their precursors, to be precise) are released into the intracellular space when the granular layer transforms into the horny one; thereafter, the liberated lipids are assembled (in the course of the enzymatically controlled process) into lamellar sheets making up the skin barrier. Standard basal therapy fails to normalize the structure of the lipid film on the skin surface. Much more efficient is the combined treatment using pharmaceutical agents that restore the hydrolipid film (emollients) and thereby ensure positive dynamics of clinical manifestations and improve characteristics of the hydrolipid skin barrier.
